Workers Choice Health Services has created a unique service to assist the physician, injured worker, employer, and case manager with the return to work process. The Care Coordinator is utilized as a liaison between these stakeholders. The Care Coordinator (often located in the physicians office) will allow for the referral to flow in a simple and systematic manner and ensure a seamless transition from the healthcare setting to the work-site. He/She is responsible for facilitating and maintaining communication, which is the hallmark of the concept.



THE ELEMENTS OF CARE COORDINATION INCLUDE:

A. Facilitate early and aggressive intervention
B. Team approach intervention
C. Facilitate relationships
D. Establish Return to Work (RTW) goals
E. Maintain communications with all parties

Using a team approach, the Care Coordinator will simplify the work injury management responsibilities of the physician and his/her staff. The care coordinator will also contact the MCO, TPA, etc. for the appropriate approvals/authorizations and will complete the C-9 if necessary. These services are provided at no cost to the physician. We take the hassle out of workers compensation and let everyone else focus on what they do best.
